Dear [FirstName],

I plan to present an essay at the 109th meeting of the American Association of Orthodontists in Boston on May 4, 2009.

Toward this goal I have prepared the "OJW* Questionnaire survey" consisting of 78 inquiries related to the safety, effectiveness, problems and benefits of Orthodontic Jaw Wiring for weight loss/control in compulsive overeating.

The central hypothesis of the research being investigated is that OJW is unsafe and ineffective.

In scientific research this is called the "null hypothesis" and is a statistical method for carrying out the investigation.

Your responses will either support the central hypothesis or indicate the contrary, viz., it is safe and effective and can be a benefit to those trying to control their weight problem.

This study will reveal how those who experienced OJW felt about its safety, effectiveness, problems and benefits.

There are seven sections: Demographics (3Qs), Background (17), Safety (11), Effectiveness (10), Problems (15), Conclusions (17) and Miscellaneous (4).

SPECIAL NOTE: The response to Q.4 re your weight, and Q.5 re your BMI at the start of OJW are noted in the top line of this email. Jot them down and place them in their respective response box.

If I haven’t received this survey back from you in thirty days I will resend it unless you "opted out."

All together there are only about one-hundred people to whom I am sending the survey.

Hopefully you will not choose to opt-out. In a study with a small sample such as this every voice counts significantly. Nevertheless, it is required to have such a link in place:

If you have not responded to my second request to complete the survey in four weeks I will assign you to the category "Non-responders" in my presentation.

Non-responders who decide to complete it at a later date are also welcome. Analysis of the responses can takes time so please submit the survey in the least delay possible.

This survey updates automatically when a responder reaches the last question and clicks on the "Done" button.

Please be frank with your responses. As the French say: "Sonner vrai comme une cloche"...just tell it like it was/is.

An honorarium of $35 will be my way of thanking you.

I will publish the results of the study  on my web site @ http://www.drted.com/OJW Questionnaire survey and results.htm

I wish to thank in advance for your participation!
